UL Lafayette students selected for verification will be notified. All requested documentation should be submitted to the Office of Financial Aid within 15 days of the request. Failure to do so may result in the reduction and/or cancellation of any need-based federal or state aid.
The verification worksheet is used to verify self-reported household size, household number in college, SNAP benefits and child support paid. Forms needed are listed under the Financial Aid section of ULink or may be printed from the Forms tab.
Acceptable IRS Documentation
To satisfy our request for IRS documentation, please read all of the options below, then select one:
IRS Data Retrieval Tool (DRT) – Unavailable until October 1, 2017 - This option is recommended by UL Lafayette and should be the option you try first. It provides you with the fastest, easiest and most secure way to verify your tax data.
IRS Tax Return Transcript – To request an IRS tax return transcript, call the IRS at 800-908-9946, complete the IRS online request form, or file IRS Form 4506-EZ. Use this option if the DRT is not available, you were not deemed eligible to use the IRS DRT, originally transferred the IRS data to the FAFSA using DRT but then updated your IRS data manually, or you filed an amended tax return (in this case, you also must submit a signed copy of IRS Form 1040X).
Signed Copy of 2015 Tax Return – As of April 24, 2017 this is acceptable documentation for the 2016-2017 and 2017-2018 award years. Returns must be signed.
